Whether you're a social worker, care coordinator or family member, we're here to make the referral process as clear and straightforward as possible.
Is Better Days Care the right fit?
We accept referrals for young adults who meet the following. If you're unsure whether someone is eligible, contact us and we'll talk it through together.
Ages 18–25
Support for young adults at the start of their adult life.
Learning Disabilities & Autism
A range of learning disabilities and autism spectrum needs.
Additional Support Needs
Including emotional wellbeing and mental health support needs.
Transition Planning
Young people moving from children's services into adult care.
What happens when you refer someone to us
Every referral is different, but the process usually follows these steps.
Initial enquiry
Contact us by phone or email with some basic information about the young person and their needs. We'll let you know quickly whether we may have availability.
Information sharing
Share relevant background information — care plans, risk assessments, health needs and any other professional reports — so we can properly consider the referral.
Assessment visit
We arrange a visit to meet the young person, and their family where appropriate, to understand their needs and check Better Days Care is the right environment for them.
Decision & agreement
We confirm whether we can offer a placement, and agree funding, start date and any transition arrangements with the referring authority.
Placement & transition
We plan a considered, unrushed move-in, with visits beforehand wherever possible so the young person feels settled from day one.
Information that helps us respond quickly
Where available, please include the following with a referral. It's fine if something isn't ready yet — we can work with you to gather what's needed.
- Referral or placement request form
- Current care or support plan
- Risk assessments
- Relevant medical and medication information
- Positive behaviour support plan (if applicable)
- Funding local authority or health board contact details
How placements are funded
Local Authority Placements
Most placements are arranged and funded through a local authority children's or adult social care team.
Health Board Funded
Some placements are funded jointly with, or fully by, a local health board where health needs are significant.
Ongoing Funding Reviews
We work with funders on regular reviews so funding continues to reflect the level of support being provided.
